At the present time the safest and healthiest way of giving a voluminous and attractive appearance to breasts which have a developmental insufficient volume or reduced after birth due to breastfeeding but not sagged too much, is to use breast prostheses.
This kind of inconveniences which might cause a loss of self-confidence especially in young ladies and later in women, are no longer a problem with the development of asthetic surgery.
As it is well known, there are two main types of breast prostheses in respect to their contents and both prostheses are covered with a thin, silicone sheeting. One of the groups of prostheses which are covered with thin, silicone sheeting containes salt water which we call physiological saline and the other group contains cohesive gel silicone.
Apart from their contents, prostheses also differ by outer surface characteristics (smooth or rough) of the silicone. If the prosthesis is to be placed under breast tissue, capsular contra-prosthesis is placed under muscle or breast tissue postoperatively by evaluating whether the patient has sufficient breast tissue after the examination and sportive activities in his daily life.
The size of the prosthesis is agreed with the patient after its properties is shared and the examinated has been made.
As it is well known, breast sizes like 75-80, 80-85 etc. are obtained by measuring the nipple and back width and subtracting the result from the measurement obtained by measuring the area under the breast and the back and are evaluated in standards stated as follows;
A cup (10 cm), B cup(16 cm), C cup(23 cm), D cup(29cm) and over them E cup
Rough-surfaced prostheses are preferred to prevent contracture development and smooth-surfaced ones are preferred because capsular contracture development is rare if the prosthesis will be placed under the muscle.
Prostheses are classified as anatomic(teardrop-shaped and round prostheses in respect to their shapes.
Anatomic prostheses create a more natural appearance by providing more fullness on the lower part of the breast. However they can not provide the sufficient fullness on the lower neck area as much as the round prostheses.
Prostheses change the cup measure and with a prosthesis application of around 125-1500cc the breast will be one cup bigger.
After the interview with the patient, the most ideal method to get the planned breast size in the operation will be testing prostheses in different sizes during the operation.
Placement of prosthesis under the breast tissue or muscle is carried out through 4 main methods. The first three of these are the more frequently used lower breast, nipple and armpit incisions.
Placement Types of Breast Prosthesis
The selection of the incision is related to the type of prosthesis to be placed, decision on whether the prosthesis will be placed below muscle or breast tissue, size of the nipple and experience of surgeon.
Breast prostheses containing physiological saline are used, though less often, with small incisions made to the belly-button.
Also, another prosthesis application method is ,if the person will undergo a abdominoplasty(abdominal lift) operation, placing the breast prosthesis through tunnels which will be arranged from the incision of this operation without making different incisions in the same session.
Breast prosthesis operation is an operation which is performed under general anesthesia, where mild pains might occur due to the stiffness of breasts and can be eliminated by analgesics and which takes 1 – 1.5 hours depending on the method applied.
After the operation, staying one night in hospital is enough, however if the operation was carried out in the morning going back home in the evening will not be a problem either.
